Position Summary

Position provides clinical social work services to referred WashU patients to complete psychosocial assessments; plans and implements needed social work services; identifies barriers to care and find solutions to the barriers; connects patients with community resources; utilizes resources available; and collaborates with healthcare team and outside agencies to provide patient-centered care.

Job Description

Primary Duties & Responsibilities:

  • Provides direct clinical social work services to referred WU patients telephonically.
  • Completes psychosocial assessments for referred WU outpatient patients.
  • Plans and implements social work services to assigned patients.
  • Completes screenings and assessment tools to meet the needs of the patient.
  • Identifies barriers to accessing care and finding solutions for those barriers.
  • Connects patients with community resources to alleviate barriers to care and improve health outcomes.
  • Utilizes resources available to patients to manage their continuum of care.
  • Collaborates with the health care team and human service agencies to meet the needs of patients and families.

Salary Range

$56,200.00 - $87,100.00 / Annually

The salary range reflects base salaries paid for positions in a given job grade across the University.
